I am a teacher at Maryborough Special School and am currently putting together an information pack for our parents/carers. Could you check to see if this information is correct and add anything else that you provide? Are you still DSQ or are you Dept of Communities?

Process for assistance within Disability Services consist of the following steps:
1. Contact or request for assistance application through your local DSQ service centre to request a service or information about services that may be available.
2. Intake- the information collected about you and your situation when you request specialist support.
3. Assessment – a standardised process to help determine eligibility and assess support needs
4. Support Linking – develop an action plan with you so that your support needs are met.
If you require further information regarding potential support services you can contact a Disability Support Service Centre.
Please see below programs that are on the Disability Services Website:
* Post School Programs
Helping young people with a disability to make the transition from school to adult life and plan to achieve future goals.
* Adult and Community Support Services
* A range of professional and specialist services to meet the needs of families supporting an individual with a disability.
* Disability Services Respite Centres
Short-term stays for adults and children with an intellectual disability.
Home and Community Care (HACC)
Provides community care services for people with a disability and their carers.
* Support for Older Carers Initiative 65+
To acknowledge the long-standing contribution of older carers to those they care for and to the community, additional respite services have been established to specifically cater for older carers and their family members.
