phoned number back, did not ring but could hear a series of clicks on the line then the sounds of a call centre in the background and someone claiming to be "Matt".

The person called regarding 'my health insurance' would not provide the name of the company they called from; they did know my name. They were fishing for details, asking Who is my health insurance provider?...'.
